TheHEALTH RECORD FORM must be filled in and returned to us before Camp.
The INDEMNITY AND RELEASE FORM must be filled in and returned to us before Camp.
The CAMP EQUIPMENT PAGE provide links to all Camps. The equipment required for each Camp is listed, and an ATHLETIC PROFILE FORM is attached, that is to be returned to the Head Coach prior to Camp beginning.
A CAR PASS that you can print has been included. There is a $10.00 one day only Gate Pass required for entry into the International Peace Garden. By showing this CAR PASS when you come to pick up your child you willbe allowed to enter the Garden at no cost.
The PUBLICITY (PHOTOS) RELEASE FORM must be filled in and returned to us befiore Camp.
