Registration Fee: $449.00 per week ($499 for Equestrian) All prices are in Canadian (CAD) Funds.

PRIVACY DECLARATION:
The Legion Athletic Camp requires the information collected on this form, and the medical form, and any other medical information subsequently provided, in order to process the athlete's registration at Camp. All information provided to the Legion Athletic Camp will be handled in accordance with Legion Athletic Camp's privacy policy. The use of the information collected on this form will be restricted to the processing of registrations, as part of applications to funders, to administer programs and activities, processing payments and payment collection. Legion Athletic Camp may use the information on this form for the purpose of gathering statistics, and reporting its activities to the public, its funders, government agencies and the Board of Directors. The information collected on the medical form, and any medical information subsequently provided, will only be used as indicated on the medical form. The Legion Athletic Camp will not otherwise disclose your personal information or the personal information of the athlete to third parties with the exception of where it is required to do so by law, or where express consent has been otherwise provided by you.

REGISTRATION NOTES
Charter buses are available from Northwestern Ontario, Weeks 1 to 4 on a space available basis at an additional cost. PLEASE INDICATE ON THE REGISTRATION FORM ABOVE IF YOU WILL BE TAKING THE CHARTER BUS. Space in some Camps is limited and placement in Camps is on a first come first serve basis.
Athletes may select only one Camp per week. The registration fee is $449 per week ($499 for Equestrian). The fee covers your accommodation, meals, coaching instruction and a FREE LEGION ATHLETIC CAMP T-SHIRT. Transportation to the Camp is the athlete's responsibility.
THE REGISTRATION APPLICATION CANNOT BE PROCESSED UNLESS THE 2010 REGISTRATION FORM (above) IS COMPLETED, DATED, SIGNED, AND ACCOMPANIED WITH A DEPOSIT OR FULL FEE PAYMENT. HEALTH AND ALL NECESSARY FORMS WILL BE SENT TO YOU WITH A FULL LEGION ATHLETIC CAMP INFORMATION PACKAGE UPON RECEIPT OF YOUR REGISTRATION FORM AND DEPOSIT OR FULL FEE PAYMENT. AN ATHLETE WILL BE CONSIDERED "COMPLETELY REGISTERED' FOR THE CAMP ONCE ALL FORMS HAVE BEEN COMPLETED, DATED, SIGNED, AND RETURNED TO THE REGISTRAR.
Outstanding balances must be paid by June 25th, 2010. NO REFUNDS paid after this date without a doctor's certificate.
A $50 cancellation fee is in effect for all registration fee refunds. NSF cheques are subject to a $30. charge. Credit cards and ATM cards not accepted.

VERY IMPORTANT INFORMATION FOR ALL ATHLETES AND PARENTS/GUARDIANS: Athletes must pass through Canadian customs on their return from Camp. It will be necessary to have proper identification. Both Canadian and American Customs have suggested a picture I.D. (current school I.D. picture card), and a birth certificate.
Don't delay, send in your registration early to reserve your spot in the Camp and Week that you want !!
