REGISTRATION NOTES
Charter buses are available from Northwestern Ontario, Weeks 2 to 4 on a space available basis at an additional cost. PLEASE INDICATE ON THE REGISTRATION FORM ABOVE IF YOU WILL BE TAKING THE CHARTER BUS. Space in some Camps is limited and placement in Camps is on a first come first serve basis.
Athletes may select only one Camp per week. The registration fee is $484 per week ($599 for Equestrian). The fee covers your accommodation, meals, coaching instruction and a FREE LEGION ATHLETIC CAMP T-SHIRT. Transportation to the Camp is the athlete's responsibility.
COMPLETE THE REGISTRATION FORM, ENCLOSING A $259.00 DEPOSIT OR FULL FEE OF $484.00 OR $599.00(EQUESTRIAN CAMP). PRINT AND COMPLETE THE CHILD SAFETY POLICY AND RELEASE. PRINT AND COMPLETE THE APPROPRIATE HEALTH RECORD FORM DEPENDING ON WHERE YOU LIVE AND MAIL TO ONE OF THE ADDRESSES BELOW AND YOU WILL BE FULLY REGISTERED FOR THE 51ST ANNIVERSARY CAMP.
Outstanding balances must be paid by June 25th, 2012. NO REFUNDS paid after this date without a doctor's certificate.
A $50 cancellation fee is in effect for all registration fee refunds. NSF cheques are subject to a $40 charge. Credit cards and ATM cards not accepted.

VERY IMPORTANT INFORMATION FOR ALL ATHLETES AND PARENTS/GUARDIANS: Athletes must pass through Canadian customs on their return from Camp. It will be necessary to have proper identification. Both Canadian and American Customs have suggested a picture I.D. (current school I.D. picture card), and a birth certificate.
Don't delay, send in your registration early to reserve your spot in the Camp and Week that you want !!
